Transmittal Form Template

Description: The Transmittal Form Template allows you to create customized transmittals. Our transmittal form is a fully editable Microsoft Excel template. There is a default logo image which can easily be replaced with your own.

Instructions: The transmittal form contains check boxes that can be selected or deselected with the click of a mouse. When opening this file be sure to enable macros, this will allow the check boxes to work properly.
